Low back
pain (Lumbago) or Low backache LBA is
commonly known as the most common pain among the other types of pain and a
primary concern in most of the patient affecting up to 80 % of the population
at approximately the age of 40 to 45 mainly, the annual The cost of Low back pain in the USA
is more than$40 billion. Traditionally it is the main concern for most of
the population worldwide because of a lack of knowledge and lifestyle.
CAUSES:
Low back pain is not primarily a disease but
symptoms that may occur concerning different causes but numbers of factors are
responsible for the causes of low back pain, especially
the mechanical cause is the central factors around which the other factors are
connected.
If the client is somehow compromising with movement difficulties
(from bio-mechanical origin) or having no proper knowledge about mechanical
advantage or disadvantage, most probably the client will complain about low back pain in his/her
day to day life.
Why my focus on lack of knowledge, because with the proper
knowledge of mechanics that is 24 hours working in body in the form of
movements, action and muscle work, we can avoid the worst complex and pain
aggravating movements.
If the client could batter understand the mechanics, what the
professionals (Physiotherapists) known as the squat lifting verses stoop
lifting and the client is not aware of these terms possibly will lead to
worsening the conditions.
In the cases of Low back pain up to 85 %
of the client with lacks of knowledge and approach to thorough clinical
examination and diagnosis possibly leads to complication of conditions and
delays in proper treatments.
This delaying in treatment shows different tissue changes (most
common tissue changes are muscular spasm, bony trauma, and changes in normal
curvature of the spine) best identifies in x-rays and physical examination.
Most accurate examination are CT scan and MRI helps to study in
detail about other tissues like discs, nerve, ligaments, these are the most
compromising tissues that show different musculoskeletal and neurological
symptoms and biomechanical disturbance (like pain in movement, tingling
and restriction of movements ) etc.
Another factor of my concern is a lifestyle, our lifestyle and
modern trends of fashions not directly but definitely lies in the causative
factors of Lowback pain,
some of the factors are:

Obesity
o
Posture
o
Physical
work
o
Long
term sitting job
o
Unhealthy
lifestyle
o
High
heels foot wares
o
Tight
clothing
o
Lack
of flexibility and strength
§ Travelling (Bike, train, crowd)
§ Some occupations (like RTO, lecturer,
housewives, laborers, farmers), etc.
§ Psychological factors (mental stress,
depression, anxiety).
As the professional knows that Low back pain covers the
lots of topic theoretically and numerous causative factors which could be
the cause of low back
pain, here I will discuss some of those terms,
1) Referred pain:- the pain arising from the pathology in the organs especially within the abdomen and pelvic region and shows the symptoms of pain in another part of the body, chest pain may also be felt in the low back area is referred pain.
2) Neurogenic Pain: - nerve compression or bulging of the disc, compression of disc and narrowing of the disc space, trauma, accident, fall from heights, etc.
3) Bony Disorders: - spondylosis, spondylitis, R.A of spine.
4) Musculoskeletal: - myofascial pain, trigger and tender points, fibromyalgia, strain, sprain, scoliosis.
5) Quada equina syndrome.
6) Tumors:- Pott’s spine

Symptoms:
First and most important symptoms are pain in the low back area at
the lumbosacral area, it may be central, unilateral or bilateral
corresponding to the area of the structure involved.
The type of pain or the sensation of pain is dull aching pain
(this type of pain is feels when it is of muscular origin). Other types of
sensation can also be feels.
The neurogenic pain is associated with tingling sensation in the
low back area, posterior of the thigh and lower leg. Pain in forward bending,
lateral bending and rotation.
Pain while carrying a load from the ground, and the activity to be
done as an extension of trunk and legs because during this movement the
compression load of 6000 N produce over this region incurred (at spine, SI
joint).

by physician)
c. Regain full-on painful ROM
d. Improve strength and flexibility

There are no specific treatment strategies or plan for all cases
of LBP, the possible treatment plan is based on proper evaluation, examination
and signs and symptoms.
Correction of causative factors is the most important part of
treatment plan.
Therapeutic aspects of treatments techniques:- (Low back pain treatment)

Mobilization
and manipulation: - The most effective technique deals with the restoration of
movement and reduction of pain. Detection of abnormalities, faulty posture,
poorly displaced anatomical tissue imbalances are corrected by this technique.

Soft
tissue therapy: - abnormalities in muscle and fascias are common compromising
structure in LBP. The compromising soft tissue structure is easily restored
with this technique
·
Neural
Mobilization: - neural tension is the prominent part of LBA. Spasm and
tightness in neural tissue produce the symptoms of numbness and progressively
tingling.

In
the acute state correction of joint and soft tissue abnormalities commonly
improves neural tension with the use of this technique (this should be strictly
done in the close supervision of the experienced clinician and PTs).

Exercise:
- strengthening and stability exercises are very effective to maintain and
improve strength and stability, they play an important role to reduce the pain
and to prevent worsening the condition. Exercise should be performed which is
advised by physiotherapists because there is no alternative to exercise. exercise
is a must in low back pain relief.

In this technological world there are numerous techniques and
exercise from general to specific but my advice to you please first consult
with professional physiotherapists because this is not a type of morning
routine and jogging exercise, this LBP could be dangerous if severed.
